With the self-declaration below (made pursuant to articles 46 and 47 of Presidential Decree 28.12.2000 n. 445) I declare under my own responsibility, aware that false declarations will be punishable under criminal law,

• 1. not to present symptoms such as cough, cold, fever associated with breathing difficulties or not
• 2. not to have stayed, even temporarily, in areas with alleged community broadcasting (widespread or local) and / or at risk of COVID-19 in the last 14 days
• 3. not to have come into contact in the last 14 days with people suspected of having contracted COVID-19 or positive to SARS-COV-2 or, in any case, even if negative, subjected to quarantine as they in turn came into contact with suspected or confirmed cases of COVID-19
• 4. that I am not currently undergoing quarantine or home isolation measures for COVID-19
• 5. not to have tested positive for the SARS-Cov-2 virus swab
• 6. not to have contracted COVID-19
• 7. or to have contracted the COVID-19 and to have carried out after the clinical recovery two swabs at distances of 24 hours, negative results respecting the period of social isolation of 14 days. I undertake to immediately notify Palakiss s.r.l. in case of any variation of one of the above conditions.
